An aneurysm is a local expansion of a cerebral vessel, namely a protrusion of a thinned area of the wall of a blood vessel. Due to the effect of arterial pressure, the size of this section (aneurysm) may increase, its thinning, which may lead to the rupture itself.

According to statistics, 1% of the world’s population has cerebral aneurysms. And this disease is a time bomb, since it is impossible to say when the rupture will occur: in the near future or in a few years.

How to diagnose an aneurysm?

To detect an aneurysm and select the correct treatment methods, it is necessary to conduct a full examination.

  • With the help of CT and MRI tomography, it is possible to quickly determine the rupture of an aneurysm, as well as to detect hemorrhage after its rupture
  • Angiography – allows you to accurately determine the location and extent of vascular damage. This study determines the location of the protrusion, its shape and size
  • Electroencephalography (EEG) is an instrumental diagnostic method that allows you to identify disturbances in the electrical activity of the brain

Aneurysm closure surgery

We would like to immediately note that there is no conservative treatment for cerebral aneurysms. Surgery is the only method, the purpose of which is to turn off the aneurysm from the general blood flow. Since the consequences of an aneurysm rupture are fatal, only surgery can save the patient’s life.

There are 2 methods of surgical intervention in the treatment of cerebral aneurysms. And both of them are considered by modern medicine to be extremely complex and quite risky, because there is a risk of damaging adjacent vessels and parts of the brain.

Types of surgery:

  • microsurgical clipping;
  • endovascular embolization

Microsurgical clipping of an aneurysm is an open microsurgical intervention. The surgeon gains access to the vessels by making a hole in the skull. Through it, he inserts a metal clip and clamps the wall of the damaged vessel. The advantage of this method is obtaining direct access to the aneurysm. Among the disadvantages is the possibility of ischemic disorders in the postoperative period.

Endovascular embolization with microspirals is a more innovative method of minimally invasive surgery, when there is no need for craniotomy. The surgeon performs the operation through the femoral artery – a single puncture of the skin in the groin area. Special catheters pass through the vessels of the femoral artery through the vessels of the neck and further into the aneurysm cavity. When the microcatheter is in the aneurysm cavity, the surgeon releases the microspiral from it. It randomly fills the aneurysm cavity in the water of the wire coil. Thus, the aneurysm filled with the microspiral is excluded from the general blood flow. Later, it is overgrown with connective tissue and no longer poses a threat to life.
During this operation, constant X-ray control is performed on a digital angiograph.

We would like to note that the final decision on the choice of the operation method is made exclusively by the surgeon, who takes into account many factors, the anatomical structure of the aneurysm, the severity of the condition, the presence of hemorrhage.
